Scope. This privacy policy covers all of TransaferHealth. It does not, however, apply to entities that TransferHealth does not own or control. By using or accessing TransferHealth, you acknowledge our privacy practices outlined herein.

Note to minors. If you are under age 18, please do not attempt to register for TransferHealth, or provide any personal information about yourself without parental agreement. If we learn that we have collected information from a minor under age 18 without parental agreement, we will work with the Provider to remove this information as quickly as possible. If you believe that TransferHealth may contain information from an unauthorized minor, please contact us at info@transferhealth.com or contact your Provider’s office directly.

1. Information We Receive

Information about yourself - when signing up for TransferHealth you provide us with various identifiable pieces of information.

Site activity information - we keep a history of some actions that take place while using TransferHealth. This may include registration histories.

Access device and browser information - when you access TransferHealth from a computer, mobile phone, or other device, we may collect information from that device about your browser type so that we may format the layout of the site to best fit the viewing area you are using. Additionally, for security purposes, we also log the IP address.

Cookie information - we use “cookies” (small pieces of data stored for an extended period of time on your computer, mobile phone, or other device) to make TransferHealth easier to use, and to protect both you and TransferHealth. You can remove or block cookies using the settings in your browser.

Web Beacons, webpages containing electronic images, are used along with cookies to compile aggregated statistics to analyze effectiveness and efficiency of our Service.

2. How We Use Your Information

We may collect and store the following information when running the service;

As is true of most websites, TransferHealth automatically gathers information about your computer such as your IP address, browser type, referring/exit pages, and operating system.

We use your information to provide and improve our Service, to administer your use of the Service, to improve your experience, and to provide updates and product announcements.

Personal Information and Files - we collect and store the data and/or files you upload with the TransferHealth Service (“Files”). TransferHealth does not sell your information or share it with third parties other than the Provider with whom you are registering for service. The storage of your information is strictly for the purpose of providing our service of registration with a third-party provider. The information stored is your information and is provided only to your Physician’s or Medical Practitioner’s office as indicated by the terms of service created and agreed to between you and your Physician’s or Medical Practitioner’s office.

Non-private or Non-Personal Information - we may disclose your non-private, aggregated, or otherwise non-personal information, such as usage statistics of our Service.

3. How We Share Your Information

Visit Registration - when you choose to register for a provider appointment through TransferHealth, we forward your information to that provider to expedite the registration for yourself and the provider. Information sent is presented to the user in the form of questionnaires prior to submission and is then captured in PDF form and granular data level form in a personal data vault only accessible by the user for future use.

All information submitted to the provider is done so based on the terms of service of the provider and acceptance of those terms by the user.

4. How You Can Change or Remove Information

Editing your submitted registration - you may change or edit your information at any time prior to completing registration either during a session or if a registration has been saved to finish later.  Subsequent to registration completion, you should contact your Physician’s office and have them return to you for further edits.

Deleting your registration - if you want to have all your information removed from TransferHealth, your Physician’s office can remove all registrations from the portal.  Please contact your Physician’s office to have them delete any current or past registrations. 

5. How We Protect Information

Steps we take to keep your information secure - we keep your account information in a secured data center with all sensitive information encrypted.  We use bank-level security to protect your data and use secure socket layer technology (SSL) with AES 256-bit encryption for all data transmission.

Risks inherent in sharing information - although we allow you to limit access to your information, please be aware that no security measures are perfect or impenetrable. We cannot control the actions of others with whom you share your information. We cannot guarantee that only authorized persons will view your information. We are not responsible for third-party circumvention of any security measures on TransferHealth. You can reduce these risks by using common sense security practices such as not leaving your registration open on your device without being present, and using up-to-date antivirus software.

Report Violations - you should report any security violations to us at info@transferhealth.com.

6. Other Terms

We may change this Privacy Policy pursuant to the procedures outlined in the TransferHealth Terms of Use. Unless stated otherwise, our current privacy policy applies to all information that we have about you and your registration. If we make changes to this Privacy Policy, we will notify you by publication in this privacy policy and on the TransferHealth website. If the changes are material, we will provide you additional, prominent notice as appropriate under the circumstances.

Business Transfers - If we are involved in a merger, acquisition, or sale of all or a portion of our assets, your information may be transferred as part of that transaction. We will notify you (for example, via email and/or a prominent notice on our website) of any change in control or use of your Personal Information or Files, or if either become subject to a different Privacy Policy. We will also notify you of choices you may have regarding the information.

Defined Terms. In this Notice, “we,” and “us,” and “TransferHealth” mean TransferHealth (a Nevada corporation) and all the officers and employees of TransferHealth who are involved in managing this website or in accessing, processing, or using non-public personal information that is placed on this website by patients. In this Notice, “you” refers to each individual patient who consents to the terms of service on this website. In this Notice, “Provider” shall mean any Physician’s or medical practices’ office that is engaged with you in the registration for an appointment at that office.
